Davis Physical Therapy LLC is offering Corporate Wellness Consulting packages to Southern WI companies. Whether your company has 1 employee or 5000 employees, it pays to invest in your employees! Working with us on a consulting basis provides you and your employees access to the knowledge and skills of a Doctor of Physical Therapy which goes beyond basic exercise classes.
Option 1: We will work with your company to identify, design, implement and evaluate an effective health and wellness program on a consulting basis and provide weekly or monthly virtual support to your employees!
Option 2: We will work with your company to identify, design, implement and evaluate an effective health and wellness program and provide in person exercise classes designed to promote camaraderie and a positive work environment on the job, all while improving the health of your employees!

Davis Physical Therapy LLC is a cash based physical therapy and wellness clinic, we do not accept or bill any insurances at this time. Payment can be in the form of cash, check or Venmo. You will be provided with a detailed invoice that could be submitted to your personal HSA or FSA account, depending on the individual requirements of your providers.

Cancellation Policy - All cancellations or rescheduling requests must be made no less than 48 hours before the scheduled appointment time in writing. By utilizing the services of Davis Physical Therapy LLC, you hereby acknowledge and agree to this cancellation policy. Cancellations or reschedule requests made with less than 48 hours written notice, but more than 24 hours written notice, may be assessed a service fee up to 50% of the scheduled service at the sole discretion of Davis Physical Therapy LLC. Cancellations or reschedule requests made with less than 24 hours written notice may be assessed a service fee up to 100% of the scheduled service at the sole discretion of Davis Physical Therapy LLC. Cancellation fees must be paid before another appointment is made. Failure to pay cancellation fees will result in a denial of services. Clients who are habitually late or cancel often may be denied services. All cancellations must be made via email or text message.
